The Maw of the Devourer
wondrous item (belt), very rare (requires attunement)
This belt is made of black leather and has a large buckle in the shape of a snarling mouth with razor-sharp teeth. The mouth is actually a portal to the realm of an ancient and insatiable entity that feeds on the life force of living creatures.
Chomp
While wearing this belt, you can also replace one of your attacks with a bite attack from the belt, using the following statistics:- Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: + your Constitution modifier piercing damage. If the target is a creature that is one size larger than you or smaller, it is grappled (escape DC 15). Until this grapple ends, you can’t use your bite on another target.
Swallow
You can use an action to swallow a creature grappled by your bite. A swallowed creature is transported through the portal in the mouth to the stomach of the ancient entity. The swallowed creature is blinded and restrained and takes acid damage at the start of each of its turns. The swallowed creature can use its action to make a DC 15 Strength (Athletics) check to escape the mouth and return to your location.Regurgitate
You can use an action to attempt to regurgitate a creature swallowed by the belt. You must make a DC 13 Constitution saving throw to do so. On a success, you expel the creature within 30 feet of you. The expelled creature is prone.Telepathy
You can use a bonus action to communicate telepathically with any creature swallowed by the belt, and you have advantage on Charisma (Intimidation) checks against swallowed creatures.Hunger
The belt must be fed every day. If you do not feed it, it will start to consume your own vitality. At the end of each long rest, if the belt has not consumed a small or larger creature, or 1 days worth of rations, you gain one level of exhaustion as it drains your life force.The entity that dwells in the belt is not loyal to you and may try to betray you or consume you if given the chance. If you die while wearing the belt, it will automatically swallow you and transport you to the entity’s realm, where you are digested and destroyed.
